diabetes education and support
Our Diabetes Education Team includes nurses who are professional Diabetes Educators, Dietitians, Podiatrists and Physiotherapists. We offer individuals with diabetes, their families, friends or caregivers an opportunity to learn about Diabetes and its management.

Services focus on client-centred care which includes comprehensive assessment, individual consultation, group programs and ongoing support.

All of our services are part of a comprehensive integrated Allied Health Team which can also refer you to other services and group activities to help improve health, wellbeing and social connection.

Service Description

The program offers individuals with diabetes, their family, friends and caregivers the opportunity to learn about diabetes self-management and gain confidence in making  healthy life style choices.  Support is available for:

  • Self-monitoring of blood glucose
  • Medication including insulin and injectable management self-monitoring of blood glucose including CGM
  • Motivational interviewing
  • Diabetes risk screening
  • Ongoing support to those who live with diabetes.

Eligibility

Referrals are required from health professional (GP /medical specialist) which include health summary medication and pathology results. Referrals  can be sent to Central Intake Service (intake1@cbchs.org.au) or via https://www.cbchs.org.au/referral/

Fees

Fees range between $10 (for Health Care and Pension Card holders) and $90 depending on your financial circumstances.
